First Person Care Clinic creating affordable health care

First Person Complete Care is a local, nonprofit medical center serving the uninsured, underinsured, undocumented, refugee, LGBT and underserve communities with an income based sliding fee scale, private insurance, Medicaid and Medicare.

First Person was able to expand their integrated services quickly through innovation by being the first
clinic to bi-directionally integrate with the HIE, the state health information exchange.
 
First Person became a valuable partner with the hospitals reducing their ER and discharge re-admission by providing a vulnerable population with a patient centered medical home, care coordination and non-medical
stress resources to bridge the patient’s gap in healthcare.

Today, First Person has expanded their locations and services, integrating behavioral care into our
primary care procedures with counselors available at our facilities to speak to the patient at their time of
need, approaching our patients with a holistic manner; understanding people are more than their
diagnosis.
 
July 2016 Huntridge Teen Clinic handed First Person CC the torch of their 22-year-old legacy and
commitment to at risk adolescents.
